What is a Medical Home?

Your Medial Home is your Primary Care Provider.  This provider may be a family doctor, nurse practitioner or clinic provider.  Having a Medical Home is important in order to monitor your total health.

Why is a Medical Home important?

Medical Homes offer treatments for short-term and chronic health problems. Have you ever worried you are at risk of having a heart attack, stroke, diabetes or cancer? Your Primary Care Provider can help prevent these diseases by monitoring your blood pressure, weight, and developing a healthy diet and exercise plan.  Your Primary Care Provider will ask you about your family history and will determine your risk for developing several serious diseases. With consistent, personalized care and regular physical exams, your Primary Care Provider can offer:

RBlood pressure/cholesterol screenings
With regular physical exams and blood tests, your Primary Care Provider can detect whether you are at risk of having a heart attack or stroke.

RChronic colds/allergy remedies
Your Primary Care Provider can help determine if harmful allergens are affecting your health. With proper guidance, you can avoid serious breathing problems that could result in Emergency Department visits.

RWeight Control
Your Primary Care Provider can offer guidelines for proper diet and exercise.  Being overweight can lead to multiple health problems including heart disease and diabetes.
