Streamlined Application & Referral
Process...
VPA offers a unique system
of coordinating care for both the patient and the provider. If
the patient is uninsured; low income (below 200% of poverty); ineligible
for Medicaid, health department services, or other available grant
programs; and in need of specialty care services they will fill out an
enrollment application, visit an eligibility specialist, receive a
Valley Project Access card, and be referred to one our volunteer
providers and/or program partners.
Valley Project
Access Qualifications...

Patient must reside in Ansonia,
Beacon Falls, Derby, Naugatuck, Oxford, Seymour or Shelton.

Patient must not have health
insurance of any kind and must not be eligible for Medicare,
Medicaid, HUSKY or VA benefits.

Patient must have an income at or
below 200% of the Federal Poverty Guideline (see chart).

Initial enrollment is for a six-month
period. An extension may be granted, pending continued eligibility
and continued treatment. However, it is the patient’s responsibility
to call Valley Project Access at (203)732-1330 to re-enroll. Hill
Health patients should call (203)503-3586.
