Dr. Lionel Lim

Our Center

The Geriatric Assessment Center at Griffin Hospital was founded by Dr. Glendo Tangarorang in 2001. The Center is now under the direction of Lionel Lim, MD, MPH. Dr. Lim attended the University of Sydney in Australia and went on to complete a residency program at New York University Downtown Hospital. Dr. Lim completed two fellowships, one in general preventive medicine and public health and the other in geriatric medicine at the Mayo Clinic College of Medicine in Rochester, Minnesota. He later served as the Medical Director of the Erlanger Bledsoe Internal and Geriatric Medicine Clinic. Additionally, Dr. Lim received his Master’s in Public Health from the University of Minnesota.

Who Will Benefit

The Geriatric Assessment Center provides an in-depth medical assessment and diagnostic service for seniors who are experiencing problems in everyday functioning. Those who benefit  from the services of a geriatric center are older persons with:
 

  • Decline in mental or physical health

  • Memory loss

  • Frequent falls

  • Gait and balance disorders

  • Behavioral changes

  • Episodes of confusion

  • Wandering

  • Safety issues

  • Incontinence

  • Multiple medication use

  • Declines in ability to care for self

  • Major changes in support needs (caregiver stress, institutional placement being considered, etc.)

  • Frequent use of the health care systems (i.e. in the last 6 months)

Our Services

The Geriatric Assessment Center provides a range of services, including:

  • “Welcome to Medicare” examination (available during the first 6 months of

  • obtaining Medicare Part B coverage)

  • Medical screening

  • Functional screening

  • Screening for depression

  • Gait and balance assessment

  • Memory impairment assessment

  • Dementia evaluation

  • Mild cognitive impairment evaluation

  • Psychosocial evaluation

  • Medication assessment

  • Treatment plan with recommendations

  • Osteoporosis screening and treatment

  • Continuity of care with a primary care physician

Seniors should be referred by their personal physician. Medicare and private insurance are accepted.
